Transition, School-to-Work, and Interagency CollaborationServices: Training on transition planning, school-to-work (models and strategies), student involvement in planning for the future, and interagency collaboration and coordination. Technical assistance is offered on pulling community constituents together, collaboration and coordination of services, problem-solving for transition and school-to-work issues, transition planning in general, personal futures planning, portfolio development, information and referral to resources and services. Also available is writing and development of resources and products in the areas of transition, school-to-work, and interagency collaboration/coordination of services. Provider: Pam Stenhjem, M.S. Audience: This service is available to any individual, organization (private or nonprofit), or department wanting assistance with these issues, both in Minnesota and nationally. In Minnesota, some services can be provided through a current contract with the State Department of Children, Families, & Learning as a free service if they are related to the work the Institute is currently under contract to provide. Nationally, services would need to be provided under an individual contract for services. Clients: Past clients have included the Minnesota Department of Human Services, Louisiana State University Medical Center, Purdue University, Minnesota Department of Education (Transition, School-to-Work, Career Development & Work-based Learning, and Special Education Offices), Minnesota Community Transition Interagency Committees, Missouri Department of Labor, National Association of Migrant Farm Workers. About the Provider: Pam Stenhjem, M.S., has been working in the field of "disability" and transition services for youth with disabilities for the past ten years. She completed a Masters of Science degree from St. Cloud State University in Applied Psychology with a focus in Rehabilitation Counseling. Pam currently works as a Grant Project Coordinator at the Institute on Community Integration in the area of transition from school to adult life for youth with all disabilities, ages 14-21. Pam has worked extensively in Minnesota's transition committees known as Community Transition Interagency Committees (CTICs). She has also worked statewide and nationally in the area of ensuring school-to-work opportunities for all learners. Her commitment is shaped through a focus on grass roots organizing, community involvement in the decision making process, and the inclusion of youth and family members as leaders in the disability reform movement.
